If you have been diagnosed with prediabetes or are at risk for developing type 2 diabetes, this program is for you. Moderate lifestyle changes and increased physical activity can cut your risk in half of developing type 2 diabetes. This program can help you learn more about how to do it! Participants work with the Lifestyle Coach and their group to develop knowledge and skills to promote sustainable lifestyle changes. Learn to read food labels The MedNetOne Diabetes Prevention Program has been recognized by the CDC as an effective lifestyle change program to prevent or delay type 2 diabetes. It is delivered by a trained lifestyle coach who understands the difficulty of lifestyle change. The core of the program includes 16 weekly sessions focused on the simple approaches to lifestyle change and their effect on preventing diabetes. Strategies included in these lessons center around skill building for healthy lifestyles, emotional cues to eating, environmental effects on lifestyle change, as well as, health and wellness related topics.
The maintenance phase includes at least six lessons that occur monthly. These lessons provide additional support and cover more specific topics such as tackling holidays and special events, time management, food preparation/recipe modification, and long-term maintenance.Date and Time
