Join us for a Ribbon Cutting celebrating Ascension - Rochester Community Primary Care!

Rochester Community Primary Care was founded in 2016. The practice started with 2 physicians, including Dr. Ryan Pradko. Ascension Providence Rochester and Ascension Medical Group, identified the need to provide primary care services to the community and what better location than adjacent to the hospital!

As the primary care need continued to grow in the community, additional providers were added. The practice now consists of 3 family medicine physicians; Drs. Ryan Pradko, Giacomo Folden, and Jade Balasz along with one women's health nurse practitioner, Nancy Mattoni.

As a result of this continued growth in the practice, Rochester Community Primary Care has recently relocated across the street from the hospital to provide the practice with a larger space to care for their patients and community.

Rochester Community Primary Care specializes in Family Medicine, offering preventative medical exams, well child visits, nutrition counseling, obesity management, women's health, and pediatric care.
